Denver Aikido LLC, dba Denver Aikikai
Agreement and Release of Liability 

This agreement is between {name} and Denver Aikido LLC, d/b/a Denver Aikikai, its instructors, members, agents, and authorized guests (collectively referred to hereafter as “Denver Aikikai”). 

In consideration for the enrollment in a martial arts program provided by Denver Aikikai, I (or my parent or legal guardian if I am a minor) agree as follows: 

1. I {name} understand that Aikido involves physical exertion and inherent risk of injury, and that participation in Aikido practices, sessions, classes, camps, seminars and other training involves aggressive physical contact with other participants. I accept and assume the risk of any injury or other medical condition or harm (including disability, paralysis, and death) arising out of or relating to my participation in any activity in which I engage at Denver Aikikai, or at any event sponsored or conducted by, through or in connection with Denver Aikikai at any other location (the “ Aikido Activities ”). I understand I am responsible for my own safety and for comporting myself in a safe manner at all times during all Aikido Activities.
   
2. If I {name} have a disability or illness, I have consulted with my physician or other medical professional before engaging in any Aikido Activities.
   
3. I {name}, and my personal representatives, heirs, and assigns, agree to indemnify and hold harmless, and hereby release, waive, discharge, and covenant not to sue  Denver Aikikai and its officers, directors, instructors, coaches, members, volunteers, guests and other participants, and  the hosts, sponsors, promoters, volunteers, and other persons involved in any Aikido Activity (collectively the “ Indemnitees ”), from any and all claims, liability, losses, damages, demands, or actions of any kind, arising out of, relating to, or alleged to be caused in whole or in part by my participation in any Aikido Activity that I may attend on or off the Denver Aikikai premises or by the action, inaction, or negligence of any Indemnitee. This includes but is not limited to claims, liability, demands or actions for personal injury or property damage or loss.
   
   
4. I {name} promise to defend, indemnify and hold Denver Aikikai harmless from any and all claims and action by third parties alleging injury from my use of any techniques or other teachings learned in any Aikido Activities. I have not requested or received any warranties as to the effectiveness of the training. 
   

I {name} have read this release and waiver of liability, assumption of risk and indemnity agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and have signed it freely and voluntarily without any inducement, assurance, or guarantee being made to me, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
